Changes to Reporting Lost, Mutilated, and Destroyed Indiana License Plates

Due to revisions in Indiana law, individuals no longer need to report damaged or destroyed commercial motor vehicle places to law enforcement agencies. Only instances involving stolen license places should be reported to the Monroe County Sheriff's Office.

If your Indiana license plate was lost, mutilated, destroyed, or stolen, complete the Indiana Bureau of Motor Vehicles form - Application for Duplicate or Replacement License Plate ( State Form 37135 )

Follow the directions at the top of the form.  If the form requires an officer’s signature (where license plates have been stolen), bring the completed form to the Monroe County Sheriff's Office during regular business hours
